    Abstract

    Three fatal engineering accidents in the past two years - the Canberra Hospital demolition that went wrong, the Westralia fire and the Esso Longford gas explosion - were investigated publicly and their causes discussed extensively at the time when each of the reports was published. Now looking at the reports together, Athol Yates found some stiking similarities in the circumstances that led to the accidents.
